Our Team Approach

• Social Counselor Our social worker deals with emotional and
  psychosocial needs of the patient and family facing terminal illness. 
  Services are coordinated with community resources for financial
  and social needs.

• Hospice Chaplain  Our Chaplain offers support services ranging
  from spiritual and grief counseling to bereavement and memorial
  services.

• Physician  Your attending physician directs your plan of care and
  remains your primary care physician.

• Nurses  A Nurse (RN or LPN) is assigned to educate the patient
  and family and to coordinate all services necessary to patient's
  illness.

• Certified Nursing Assistants  To assist patient and family with
  personal care and activities of daily living such as bathing, feeding
  and hands on care.

• Therapy  Including Physical Therapy, Occupational Therapy, and
  Speech Therapy. 

• Hospice Volunteers  Volunteers provide the extra love and care that
  our patient's and families need. Volunteers offer companionship and
  practical, caring help.
