Social Counselor
Our social worker deals with emotional and

Services are coordinated with community resources for financial
and social needs.
Hospice Chaplain Our Chaplain offers
support services ranging
from spiritual and grief counseling to bereavement and memorial
services.
Physician Your attending physician
directs your plan of care and
remains your primary care physician.
Nurses A Nurse (RN or LPN) is assigned
to educate the patient
and family and to coordinate all services necessary to patient's
illness.
Certified Nursing Assistants To assist
patient and family with
personal care and activities of daily living such as bathing, feeding
and hands on care.
Therapy Including Physical Therapy,
Occupational Therapy, and
Speech Therapy.
Hospice Volunteers Volunteers provide
the extra love and care that
our patient's and families need. Volunteers offer companionship and
practical, caring help.
